Spam is showing up in SpamButcher's 'Intercepted Spam' window, but that same spam makes it to my inbox.

Sometimes having your e-mail client setup to leave messages on the server can prevent SpamButcher from killing spam. If your client is setup this way - you may want to change it to help improve SpamButcher's spam blocking performance. In most e-mail clients this setting can be accessed by opening your account settings and then clicking on the "advanced" tab.

If you use web-based-email this problem can be caused by leaving your webmail access page open in the background. Try closing your webmail account unless you are actually reading email.
